Fill out this Google Form to begin the process indicating your chosen illness or if you'd like to be assigned one. Research and learn about the illness and provide relative information in your project such as symptoms, causes, prevention, treatment, etc.

Design a creative method of informing others about your chosen uncommon illness. You are free to design your project through any platform in a professional manner. Platforms include but are not limited to Google Docs, Prezi, digital posters, Tik Tok, Instagram, videos, etc.  Refer to this Google Doc which includes the requirements needed to complete a project.       *

 You will receive a confirmation email on whether or not your project meets the requirements. If it does, you will also receive a link to another Google Form that you must fill out to ensure that you we have record of your project and how long it took to complete your project.

Your project will then be published on this website and our social media pages which will fully credit all your hard work! Please feel free to send in as many completed projects as you desire!
